The Ohio Department of Insurance has released the Proof of Claim Form and Instructions for those who wish to file a claim for unpaid balances related to Thin Blue Line Insurance.  This form is different from the complaint you may have filed already. This is the next step in the process.  If you believe you are entitled to reimbursement or payment from Thin Blue Line, please review all attachments.  The deadline for filing the Proof of Claim Form and related documentation is August 31, 2026.
